First Visit
Everyone enjoys their first visit to Konikoff Dentistry
Picking up the telephone and scheduling an appointment with a new dental office can be very difficult. But once you speak with our patient coordinators, you will immediately know that you made the right decision. After we have asked some general information of you, we will send you a NEW PATIENT packet. This packet consists of some necessary forms that will need to be completed by you to enable us to give you the best of care at your visit.
Next, we will schedule you an initial appointment with one of our skilled hygienists who will do an assessment of your oral health. At the end of your appointment, a doctor will be in to do an exam. Depending on your level of oral health, some dental treatment may be performed the same day as your initial appointment. From there, additional visits may be needed to complete any treatment needed.

Forms and Medical Records
X-RAYS We pride ourselves in delivering the highest standard of care; therefore, complete diagnostic x-rays are necessary. We require a complete series of x-rays on new patients and patients who have not been to see us on a regular basis. If you have had this series done with another dentist in the past three years, we ask that you bring them with you on your initial visit. If you do not have them or are not able to retrieve them from your prior dentist before your appointment with us, we will need to take x-rays and bill you accordingly.
MEDICAL INFORMATION Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
HIPAA FORM (completed) Konikoff Dentistry works diligently to abide by the federal regulations imposed for the privacy and security of patient records and information.
Alert to Parents
A parent or legal guardian must accompany all children under the age of 18 at the first visit and for any ensuing dental visits where treatment by the dentist will occur. |
